Mendota office: 901 Main St, Mendota, IL 61342. Appointments Tuesday through Thursday, 8 AM to 8 PM. Our phone lines are open Monday through Friday, 9 AM to 5 PM. The office is by appointment only, so call (815) 993-1614 or request a call back to schedule.
